INFLUÊNCIA DO POSICIONAMENTO EM PREMATUROS INTERNADOS EM UNIDADE DE TERAPIA INTENSIVA: UMA REVISÃO DE LITERATURA

Introdução: Segundo a OMS, a prematuridade representa um problema de saúde pública interferindo na taxa de morbi-mortalidade. A prematuridade implica em longos períodos de internação em unidades de terapia intensiva. Esses bebês internados passam por diversos procedimentos e manuseios. Desse modo, o posicionamento terapêutico é utilizado como estratégia para minimizar as complicações advindas desses procedimentos e do período de internação. Objetivo: Verificar por meio da literatura cientifica como o posicionamento interfere na rotina e no desenvolvimento do prematuro internado em unidade de terapia intensiva neonatal. Metodologia: O estudo tratou-se de uma revisão de literatura realizada no período de janeiro a maio de 2020 nas bases de dados Science Direct, SciELO; Portal BVS; PEDro, PubMed. Resultados: Foram incluídos e analisados no estudo doze artigos que relacionavam o posicionamento como estratégia terapêutica e como isto influenciava no ambiente de terapia intensiva. As posturas utilizadas foram decúbito prono, supino e decúbitos laterais. Discussão: Observou-se que a posição prona aumentou a atividade parassimpática, reduziu dor, estresse, tempo de choro e cortisol salivar e menor volume residual gástrico. A posição supina caracterizou maior nível residual gástrico e menor atividade parassimpática, quando comparada a prona. Na posição supina ocorre maior excitação e gasto energético. A posição lateral é considerada insegura, pois favorece a diminuição do calor corporal e reinalação dos gases expirados, podendo levar a morte súbita. Observou-se também que na posição lateral os pré-maturos obtiveram menor nível de pepsina do aspirado traqueal, redução de resíduos gástricos e frequência cardíaca media elevada. Considerações finais: observou-se que os posicionamentos de supino, prono e decúbito lateral podem interferir em condições que variam desde o quadro clinico até o neurodesenvolvimento do prematuro.

    Introduction: According to the WHO, prematurity represents a public health problem that interferes in the rate of morbidity and mortality. Prematurity implies long periods of hospitalization in intensive care units. These hospitalized babies go through various procedures and handling. Thus, therapeutic positioning is used as a strategy to minimize complications from these procedures and the length of hospital stay. Objective: To verify through the scientific literature how positioning interferes with the routine and development of premature infants admitted to a neonatal intensive care unit. Methodology: The study was a literature review carried out from January to May 2020 in the Science Direct, SciELO databases; VHL Portal; PEDro, PubMed. Results: Twelve articles were included and analyzed in the study that related positioning as a therapeutic strategy and how it influenced the intensive care environment. The postures used were prone, supine and lateral decubitus. Discussion: It was observed that the prone position increased parasympathetic activity, reduced pain, stress, crying time and salivary cortisol and reduced gastric residual volume. The supine position characterized a higher gastric residual level and less parasympathetic activity when compared to the prone. In the supine position there is greater excitement and energy expenditure. The lateral position is considered unsafe, as it favors the reduction of body heat and the rebreathing of expired gases, which can lead to sudden death. It was also observed that in the lateral position, the preterm infants obtained a lower level of pepsin from the tracheal aspirate, reduced gastric waste and high average heart rate. Final considerations: it was observed that the supine, prone and lateral decubitus positions can interfere in conditions that vary from the clinical condition to the neurodevelopment of the premature infant.
