RELATO DE CASO: PACIENTE COM EPILEPSIA REFRATÁRIA ASSOCIADA A OLIGODENDROGLIOMA

O oligodendroglioma é uma forma rara de tumor cerebral primário, responsável por cerca de 2-5% dos tumores intracranianos, sendo mais comum em homens adultos, mas raro em crianças, representando cerca de 1-2% de todos os tumores cerebrais primários em crianças. É originário de oligodendrócitos ou células precursoras da glia e exibe crescimento infiltrativo e lento. Eles podem ser classificados em dois graus (Grau II e Grau III), onde o Grau II são tumores de baixo grau e têm células tumorais que crescem lentamente e invadem o tecido normal próximo, e o Grau III é maligno e tem tumores em rápido crescimento. Tem baixo crescimento e 50-80% dos casos o primeiro sintoma é convulsões. Uma convulsão é um sintoma que requer uma solução rápida que geralmente requer tratamento empírico no momento inicial para interromper as convulsões e promover algum bem-estar para a rotina do paciente. As convulsões podem causar muitos danos à vida do paciente, especialmente se o paciente é uma criança que não sabe o que fazer e o que fazer. Dependendo da intensidade e frequência dos episódios convulsivos, o paciente pode sofrer fraturas por quedas, sofrer discriminação e preconceito na escola. As causas da epilepsia refratária são diversas; portanto, sempre que um paciente é refratário ao tratamento medicamentoso, a origem das convulsões deve ser investigada precocemente, não basta parar o tratamento empírico.

    Oligodendroglioma is a rare form of primary brain tumor, accounting for about 2-5% of intracranial tumors, being more common in adult males but rare in children, accounting for about 1-2% of all primary brain tumors in children. It originates from oligodendrocytes or glial precursor cells and exhibits infiltrative and slow growth. They can be classified in two grades (Grade II and Grade III), where Grade II are low grade tumors and have tumor cells that grow slowly and invade nearby normal tissue, and Grade III are malignant and have rapidly growing tumors. It has low growth and 50-80% of cases first symptom is seizures. A seizure is a symptom that requires a rapid solution usually requiring empirical treatment at the initial time to cease seizures and promote some well-being for the patient's routine. Seizures can cause a lot of damage to the patient's life, especially if the patient is a child who does not know what to do and what to do. Depending on the intensity and frequency of seizure episodes, the patient may experience fractures with falls, may suffer discrimination and prejudice at school. The causes behind refractory epilepsy are diverse, so whenever a patient is found to be refractory to drug treatment, the origin of seizures should be investigated early, it is not enough to stop at empirical treatment.
