EFETIVIDADE DA PRÉ-REABILITAÇÃO SOBRE OS DESFECHOS PRÉ E PÓS-OPERATÓRIOS DE PACIENTES SUBMETIDOS À CIRURGIA CARDÍACA: UMA REVISÃO INTEGRATIVA

Introdução: A pré-reabilitação (PR) é uma intervenção realizada antes de procedimentos cirúrgicos e envolve principalmente: exercício físico, cuidados nutricionais e psicológicos. Essa abordagem visa aumentar a capacidade funcional e melhorar a saúde do indivíduo, de modo a reduzir a morbimortalidade pós-operatória e potencializar a reabilitação. A literatura já apoia o uso da PR na oncologia, entretanto, informações sobre esse tipo de intervenção na cardiologia são pouco conhecidas. Desse modo, esta pesquisa objetivou revisar as evidências disponíveis na literatura sobre os efeitos da PR nos desfechos perioperatórios de pacientes submetidos à cirurgia cardíaca. Metodologia: Realizou-se uma revisão integrativa, em julho de 2020, nas bases de dados EMBASE, MEDLINE, PEDro, Scopus e Google Scholar. Os termos usados foram: “Prehabilitation” AND “Cardiovascular Surgical Procedures” OR “Cardiac Rehabilitation” OR “Preoperative Care” OR “Postoperative Complications” OR “Treatment Outcome”. Não houve restrição de ano ou idioma. Incluíram-se pesquisas com seres humanos, excluindo-se artigos de revisão, protocolos de pesquisa, estudos sem relação com o tema ou indisponíveis.  Resultados e discussão: Foram identificados 1.403 resultados, destes, quatro atenderam aos critérios de elegibilidade. Ao todo, 257 voluntários participaram das pesquisas que envolveram pacientes no pré-operatório de transplante cardíaco, revascularização do miocárdio e substituição valvar. Foram evidenciados resultados estatisticamente significativos (p<0,05) para o aumento da capacidade funcional; diminuição dos níveis de ansiedade, estresse e depressão; redução da intensidade álgica pós-operatória e do tempo de internação hospitalar. Nenhum estudo relatou intercorrências cardíacas durante a PR. Considerações finais: As evidências atuais são promissoras, pois sugerem que a PR cardíaca possa melhorar o desempenho físico e a saúde global dos pacientes submetidos à cirurgia cardiovascular. Entretanto, mais pesquisas são necessárias para que se possa reforçar ou refutar os benefícios atribuídos a esse tipo de intervenção.

    Introduction: Pre-rehabilitation (RP) is an intervention performed before surgical procedures and mainly involves: physical exercise, nutritional and psychological care. This approach aims to increase the functional capacity and improve the health of the individual, in order to reduce postoperative morbidity and mortality and enhance rehabilitation. The literature already supports the use of RP in oncology, however, information about this type of intervention in cardiology is little known. Thus, this research aimed to review the evidence available in the literature about the effects of RP on the perioperative outcomes of patients undergoing cardiac surgery. Methodology: An integrative review was carried out, in July 2020, in the EMBASE, MEDLINE, PEDro, Scopus and Google Scholar databases. The terms used were: “Prehabilitation” AND “Cardiovascular Surgical Procedures” OR “Cardiac Rehabilitation” OR “Preoperative Care” OR “Postoperative Complications” OR “Treatment Outcome”. There was no year or language restriction. Research with human beings was included, excluding review articles, research protocols, studies unrelated to the topic or unavailable. Results and discussion: 1.403 results were identified, of these, four met the eligibility criteria. In all, 257 volunteers participated in research involving patients in the preoperative period of heart transplantation, myocardial revascularization and valve replacement. There were statistically significant results (p <0.05) for increasing functional capacity; decreased levels of anxiety, stress and depression; reduction in postoperative pain intensity and length of hospital stay. No study reported cardiac complications during RP. Final considerations: Current evidence is promising, as it suggests that cardiac RP may improve the physical performance and overall health of patients undergoing cardiovascular surgery. However, more research is needed to reinforce or refute the benefits attributed to this type of intervention.
