PANORAMA DO CÂNCER DE ESÔFAGO NO BRASIL: UMA ABORDAGEM ACERCA DOS ÍNDICES DE MORTALIDADE ENTRE 2010 E 2017

RESUMO: Câncer esofágico (CAE) é uma neoplasia severa e de alta prevalência, sendo o 3º tumor maligno mais comum no sistema gastrointestinal em todo o mundo. O presente trabalho tem como objetivo delinear o perfil epidemiológico dos óbitos por câncer de esôfago a nível nacional entre 2010 e 2017. Consiste em um estudo transversal, descritivo e retrospectivo a partir de dados do Instituto Nacional do Câncer. Foram utilizados os seguintes descritores: óbitos, região, taxas de mortalidade (a cada 100 mil habitantes), sexo e faixa etária. No período em análise, o câncer de esôfago foi responsável por 64.355 óbitos, sendo a taxa de mortalidade nacional (3,89 a cada 100 mil) superior à média mundial (3,71 /100 mil). Quanto à distribuição regional das notificações, o Sudeste despontou com o maior número de registros (30.568 óbitos), sendo seguido pelas regiões Sul (15.777), Nordeste (12.494), Centro-Oeste (3.815) e Norte (1.701). Em relação ao sexo, os homens demonstraram índices mais exuberantes, perfazendo cerca de 78% dos óbitos e taxa de mortalidade (6,67 /100 mil) significantemente maior que a encontrada entre as mulheres (1,57 /100 mil). A faixa etária e sexo são descritas como predominantemente na terceira idade de população do gênero masculino, o que de fato condiz com os dados demonstrados, cerca de 63% dos óbitos constatados nos últimos 10 anos, apresentam paciente com mais de 60 anos, juntamente com o sexo masculino, representando 78% dos casos. Notou-se, ainda, a existência de maiores índices em pacientes com idade elevada e taxa de mortalidade nacional superior à mundial. Diante desse cenário, estima-se que a atuação de políticas públicas voltadas à conscientização e manejo dos fatores de risco modificáveis projeta-se como importante mecanismo para viabilizar a redução da mortalidade pelo agravo em tela.

    ABSTRACT: Esophageal cancer (CAE) is a severe and highly prevalent neoplasm, being the 3rd most common malignant tumor in the gastrointestinal system worldwide. The present study aims to outline the epidemiological profile of deaths from esophageal cancer at the national level between 2010 and 2017. It consists of a cross-sectional, descriptive and retrospective study based on data from the National Cancer Institute. The following descriptors were used: deaths, region, mortality rates (per 100 thousand inhabitants), sex and age group. In the period under analysis, esophageal cancer was responsible for 64,355 deaths, with the national mortality rate (3.89 per 100 thousand) higher than the world average (3.71 / 100 thousand). As for the regional distribution of notifications, the Southeast emerged with the largest number of records (30,568 deaths), followed by the South (15,777), Northeast (12,494), Central-West (3,815) and North (1,701) regions. Regarding gender, men showed more exuberant rates, making up about 78% of deaths and mortality rate (6.67 / 100 thousand) significantly higher than that found among women (1.57 / 100 thousand). The age group and sex are described as predominantly in the third age of the male population, which in fact is consistent with the data shown, about 63% of the deaths recorded in the last 10 years, present a patient over 60 years old, together with the male sex, representing 78% of the cases. It was also noted the existence of higher rates in patients with high age and a higher national mortality rate than the world. In view of this scenario, it is estimated that the performance of public policies aimed at raising awareness and managing modifiable risk factors is projected as an important mechanism to enable the reduction of mortality from the disease on screen.
