RESSECÇÃO DE CONGLOMERADO LINFONODAL E METÁSTASE HEPÁTICA DE TUMOR MISTO DE TESTÍCULO: RELATO DE CASO E REVISÃO DE LITERATURA

Paciente masculino de 23 anos com dor testicular direita, submetido a avaliação radiológica e laboratorial com indícios de tumor de testículo. Estadiamento demonstra metástase para linfonodos de retroperitônio, pulmão e fígado. Submetido a abordagem multidisciplicar com orquiectomia, seguida de quimioterapia, ressecção de conglomerado linfonodal e de metástases hepáticas. A ressecção das metástases hepáticas só foi possível utilizando-se da ligadura do ramo direita da veia porta com consequente hipertrofia do lobo esquerdo do fígado (cirurgia à ALLPS). Após o procedimento cirúrgico houve a necessidade de tratamento endoscópico de fistula biliar com envolvimento de mais uma equipe médica. Paciente apresentou boa evolução pós operatória aguardando ressecção das metástases pulmonares. Grande parte dos pacientes com tumor de testículo são homens jovens que, ao diagnóstico já apresentam doença metastática, mais comumente em linfonodos, pulmão e fígado. A ressecção das metástases restantes após realização de quimioterapia deve ser encorajada devido a boa sobrevida dos pacientes após controle total da doença, por mais que o tratamento esteja atrelado a taxas de morbimortalidade consideráveis. O tratamento das metástases de tumor de testículo é desafiador, complexo e multidisciplinar. Somente com o envolvimento de várias equipes com alto nível técnico é que se pode prestar a assistência demandada por estes pacientes ao ponto de se alcançar bons resultados.

  • Abstract:

    Male patient, 23 years old with right testicular pain, submitted to radiological and laboratory evaluation with evidence of testicular tumor. Staging demonstrates metastasis to retroperitoneum, lung and liver lymph nodes. He was underwent a multidisciple approach with orchiectomy, followed by chemotherapy, resection of conglomerate lymph node and liver metastases. The resection of liver metastases was only possible using the right ligadure of the right branch of the port vein with consequent hypertrophy of the left lobe of the liver (ALLPS surgery). After the procedure, there was a need for endoscopic treatment of the biliary fistula with the involvement of another team. Patient shows good postoperative evolution awaiting resection of lung metastases. Most of the patients with testicular tumor are young men who, at diagnosis, have already detected metastatic disease, more common in lymph nodes, lungs and liver. The resection of the remaining metastases after undergoing chemotherapy should be encouraged due to the good survival rate of patients after total control of the disease, even though the treatment is associated to considerable morbidity and mortality rates. The treatment of testicular tumor metastases is challenging, complex and multidisciplinary. Only with the involvement of several teams with high technical level, the pacience can achieving good results of the treatment.
