PROTOCOLO DE REFERENCIAMENTO DE PACIENTES DOMICILIARES PARA ASSISTÊNCIA MULTIPROFISSIONAL ATRAVÉS DO APOIO MATRICIAL DO NASF DO MUNICÍPIO DE SERRINHA-BA
Os Núcleos de Apoio à Saúde da
Família (NASF) têm em sua organização uma
equipe multiprofissional que deve atuar de
maneira integrada e apoiando os profissionais
das equipes de Saúde da Família e das equipes
de Atenção Básica. O objetivo do presente
trabalho é consolidar o apoio matricial da equipe
do NASF na assistência domiciliar através da
criação de um Protocolo de Referenciamento
de Pacientes Domiciliares para Assistência
Multiprofissional. Trata-se de um relato de
experiência da criação de um protocolo
multiprofissional partindo da identificação
do processo de trabalho entre as eNASF e
eESF e seguido da elaboração do protocolo
pela eNASF com as ações de Educação
Permanente em Saúde. A base téoricometodológica
para a criação do protocolo
resultou da coleta de informações através das
publicações do Ministério da Saúde de 2008
à 2018, assim como do Caderno de Atenção
Básica nº 27 e o Caderno de Atenção Básica
nº 39. A elaboração do protocolo possibilitou
a organização do processo de trabalho,
conduzindo o planejamento da assistência
domiciliar e o aperfeiçoamento do cuidado
continuado compartilhado entre as equipes.

Abstract:
The Family Health Support
Centers (NASF) have in their organization
a multiprofessional team that must act in an
integrated manner and support the professionals
of the Family Health teams and the Primary
Care teams. The objective of the present study
is to consolidate NASF team support in home
care through the creation of a Referral Protocol
for Home Care Patients for Multiprofessional
Assistance. This is an experience report of
the creation of a multiprofessional protocol
based on the identification of the work process
between eNasf and eESF and followed by the
elaboration of the protocol by eNASF with the
actions of Permanent Education in Health.
